Your First Visit
Getting started with Lake City OBGYN
Congratulations! You have joined the Lake City OBGYN family and are about to have your first appointment. To make sure your first appointment is enjoyable and stress-free, we ask that new patients go through the list below BEFORE their appointment. Before you come in, we will ensure that your insurance coverage is accurate and inform you of any deductible, co-insurance, and co-pay estimated amount that may be incurred.
Please go through the list of actions below before coming to your first appointment. Questions? Call us at 386-438-5095.
1
Print out our New Patient Forms and fill them out beforehand
You can save time by printing and filling out our patient forms and bringing with you to your first visit.
2
Bring your current medication bottles with you to your first appointment.
3
Prepare a list of questions or issues that you would like to discuss with your physician or midwife.
4
Bring your insurance card to your first appointment.
Your First Annual Exam
During your visit, the physician or midwife will first meet with you to talk about your history including:
- Your periods – when did they start, their duration, and any symptoms you have before or during
- Your pregnancy history
- Your sexual history
- Lifestyle issues that affect your health like smoking, your diet, and your exercise habits
Once you are ready, the nurse will help to make you comfortable in an exam room and show you how to put on a patient gown. The physician or midwife will explain what will be done prior to any procedure and will make every effort to ensure that you remain as comfortable and relaxed as possible.
After your exam, you may dress in private and the physician or midwife may meet with you again to ensure all your needs have been met and your questions have been answered.

Your First OB Visit
For most women, this is a Pregnancy Confirmation Visit. The first visit with your physician or midwife will include:
- Confirmation by urine testing that you are pregnant
- An ultrasound by your physician or midwife to see how far along you are
- Samples of, and a prescription for, prenatal vitamins
- Laboratory testing orders which can be either:
- Done at our office for a nominal convenience fee or
- Completed at the laboratory prior to your Ob visit
- An appointment for your first Ob visit with your physician or midwife of choice
Your First Procedure Visit
Your physician will have ensured that you are confident that the planned procedure is right for you, and that you understand all the benefits, other options for treatment, and risks of the procedure. This is called “informed consent.” Prior to this visit, you will also be given:
- Information on any financial responsibility on your part for the procedure
- A prescription for relevant medications
- Pre-operative instructions in writing and verbally
- An order for any necessary laboratory studies
- Post-operative instructions
- A appointment for a postoperative visit after your procedure
